Clinics and health practitioners

QR codes in clinics

The intake form, filled in before they walk in.

Appointments, history and follow-up, without anyone dictating anything over the phone.

QR code that opens the QRlify generator
Scan it — it is a real one
The QRlify generator showing a clinic’s use cases: book an appointment, intake form, professional profile, patient information, follow-up instructions and clinic WhatsApp
You pick what it is for, not between eighteen types of code.
Six uses

Six places to put it

In the waiting room, on the appointment card, on the prescription. Each one with its own code and its own numbers.

01

Book an appointment

Your calendar, without the round of calls and messages.

02

Intake form

History and consent, filled in before they sit down.

03

Professional profile

Training, specialty and registration, on one page.

04

Patient information

What you explain ten times a day, in a PDF they can reread.

05

Follow-up instructions

Dosage and schedule, without relying on them keeping the paper.

06

Clinic WhatsApp

Direct contact, with the reason already written.

Why dynamic

The difference starts at the printer

A plain QR code

The address lives inside it

You change calendar tool, phone number or address, and the cards you handed out and the sign on the door lead to where you no longer are.

And you never learn how many people used it, because it does not pass through anywhere that could count.

A dynamic QR code

We keep the address for you

The code points at QRlify and QRlify points wherever you say. Change the destination whenever you like and whatever you printed still works.

And because every scan passes through here, it can be counted.

What you see the next morning

How many scans, and on which days. That shows whether the appointment card gets used or whether people still just call.

The day-by-day and per-campaign detail starts on the Starter plan. The scan count is on every plan.

How it is done

Four steps and it is in the waiting room

You say what you do

Pick «Businesses and organisations» and then «Health and private practice». That is enough for us to know what to offer, so you never choose between eighteen types of code.

You pick what it is for

Appointments, intake form, profile, information, follow-up or WhatsApp. Each one arrives with its field ready.

You give it your colours

Two colours and, on a paid plan, your logo in the middle. Black on white also works, and scans just as well.

You download and print it

PNG or SVG, at whatever size you need. From then on, anything that changes you change from the dashboard.

A page, not a link

All of it together, in a QCard

A QR code leads to one place. A QCard is a page of yours with several buttons: booking, the intake form, how to find you, your WhatsApp and the handouts you usually give.

You point a single code at that page and change the buttons whenever you like. A change of calendar or address does not mean reprinting cards.

A clinic QCard on a phone: practitioner name, description and buttons to book an appointment, the intake form, how to find the clinic and WhatsApp
Questions

What clinics usually ask

Yes, by pointing at your form. The usual place is the appointment card or the reminder, so it arrives already filled in. If you change tools, you change the destination and what is printed still works.
No. It records the scan — when and from what kind of device — so it can be counted, and nothing else. Whatever you collect in your form lives in the tool you use for that, not here.
Yes, if the code is dynamic. You change the destination from the dashboard and the cards you already handed out still lead to the right number.
No. The camera on any modern phone reads the code directly.
Creating and downloading a static code is free and unlimited. The dynamic one — the one you can change and measure — starts on the free plan. See the plans.

The calendar changes. The card does not.
